Recently, the power industry has seen a series of accidents (incidents) caused by "three violations" (illegal command, illegal operation, and violation of labor discipline), with extremely tragic consequences and extremely typical problems exposed. On June 24, during the power transmission of the 500 kV Fengzhang II line of the State Grid Henan Electric Power Company, the dispatcher missed the order and did not open the grounding knife switch on the power plant side. The operation and maintenance personnel did not put the line protection on the Zhangde transformer side into operation during the maintenance and operation of the Fengzhang II line, resulting in a three-phase short circuit and grounding when the line was powered on, and the backup protection tripped across the level, causing the 500 kV Zhangde substation, 4 220 kV substations, and 1 railway traction station to lose pressure. On June 29, a staff member of Shaanxi Huadian Yuheng Coal and Electricity Company did not restore the safety measures for the press rails during the commissioning of the B vacuum dehydrator, and did not take measures to stop the running press rails. He was caught in the running press rails and died. Another worker blindly tried to rescue the victim without stopping the pressure rail, and his head collided with the running pressure rail, causing the death toll to rise to 2. On July 13, a worker who was not wearing a safety belt or a helmet fell to his death while climbing down the ladder from the third tower of the No. 159 wind turbine in a multi-energy complementary construction project of the China Coal Power Hami Dananhu New Energy Company.

In order to learn lessons from accidents (incidents) and effectively curb the frequent occurrence of power accidents (incidents) caused by "three violations", the National Energy Administration decided to launch a three-month special campaign against "three violations" in power production safety across the country from August to October 2024. The relevant matters are hereby notified as follows.

1. Further strengthen learning and establish a safe development concept. All units should effectively improve their political stance, conscientiously organize the study of General Secretary Xi Jinping's important instructions on production safety, further enhance their ideological understanding, do a good job in production safety with an extremely responsible spirit for the people, and firmly hold the red line that development must not be at the expense of human life. We must strictly implement the requirements of relevant policy documents on power production safety, overcome paralysis and fluke mentality, further consolidate production safety responsibilities, ensure that safety investment, safety training, basic management, and emergency rescue are in place, and include subcontracting units, outsourced personnel, outsourcing teams, and labor dispatch personnel into the enterprise's production safety management system. In accordance with the principle of "four no-pass", we must conduct a comprehensive and in-depth investigation of the power accidents (incidents) that have occurred, conscientiously summarize and reflect, dig deep into the root causes of the accidents, and take practical and effective measures to truly implement all requirements for power production safety and implement them on the front line.

2. Organize actions quickly to comprehensively investigate and rectify the "three violations". All power companies should learn lessons from accidents (incidents), quickly organize a special action against the "three violations", carry out special rectification of various illegal commands, illegal operations, and violations of labor discipline, and comprehensively investigate production and construction plans, equipment operation status, and personnel control. We must take the "two tickets and three systems" as a starting point, effectively grasp the on-site control of production operations, hold "pre-shift meetings" and "pre-work meetings", strengthen the analysis and pre-control of dangerous points, and increase the investigation and punishment of stubborn violations such as unlicensed operations, not wearing safety helmets, not wearing safety belts, not implementing safety measures, misoperation, and blind rescue. We must conduct special research and analysis on typical violations and outstanding problems in the links and positions where violations occur frequently, and explore the establishment of an effective prevention mechanism to eliminate violations.

3. Strengthen publicity, education and training to enhance the safety awareness and capabilities of frontline employees. All power companies should adhere to the principle of "downward, downward, and downward again, implementation, implementation, and implementation again", and conduct an "anti-three violations" warning education activity covering all employees, especially grassroots teams and frontline personnel during the special action period, to strictly prevent habitual violations. It is necessary to flexibly use a variety of means such as public accounts, short videos, and news reports to widely publicize the spirit of General Secretary Xi Jinping's important expositions on production safety, laws and regulations on power production safety, and key points of power production safety skills. It is necessary to adhere to the principle of "training before taking up the post", strengthen education and training for new employees, transferred employees, employees in high-risk positions, safety management personnel, etc., encourage workshops and teams to sort out and post typical violation performance databases of their professions and positions, help master the necessary production safety skills, and cultivate and establish correct safety values.

4. Effectively implement the requirements for anti-accidents and establish a sound working mechanism. All power companies should carefully study the national power production safety situation report issued monthly by the National Energy Administration, and refer to the anti-accident measures such as the "Twenty-five Key Requirements for Preventing Power Production Accidents", "Thirty Key Requirements for Preventing Power Construction Project Construction Safety Accidents", and "Electricity Safety Work Regulations", to find the weak links in their own production safety work and propose improvement measures. It is necessary to conduct a complete review of the unit's safety management system, regulations and standards, and safety risk control mechanisms, focusing on whether the safety risk identification and control are complete, whether there is a "two-skin" phenomenon between the work site and the rules and regulations, etc., and timely optimize and revise them to establish a sound normalized working mechanism.

5. Increase the intensity of punishment and establish a sound reward and punishment mechanism against the "three violations". All power companies should seriously investigate and correct all kinds of violations, study and establish a violation points mechanism, and resolutely punish operators who do not conscientiously implement safety management regulations, fail to implement safety construction plans, have serious violations, and have reached the upper limit of violation points in accordance with regulations. If it involves outsourcing units and outsourced personnel, they must be ordered to suspend work and rectify until they are cleared out. It is necessary to change the single method of directly rewarding and punishing outsourcing units, and implement the reward and punishment results to specific construction teams and specific operators to improve the pertinence and effectiveness of rewards and penalties. In light of actual conditions, it is necessary to regularly carry out activities such as "no violation teams" and "no violations around party members" to increase positive publicity and positive incentives for compliance with rules and regulations.

6. Strengthen organizational leadership and supervision and inspection, and summarize and improve in a timely manner. After receiving this notice, all power companies should immediately make special arrangements for the special action against the "three violations", effectively strengthen organizational leadership, and ensure the implementation of the work. All enterprise member units of the National Electricity Safety Committee shall submit a summary of the special action to the Electricity Safety Supervision Department of the National Energy Administration before November 15, 2024. All dispatched agencies and local power management departments should, in conjunction with various regulatory work, conduct irregular supervision and inspection of the special action against the "three violations" of power companies within their jurisdiction, and report major situations in a timely manner. In particular, it is necessary to strengthen the guidance and inspection of local power companies and small and medium-sized power companies to help improve the level of production safety. After the special action is over, each unit should continue to carry out anti-"three violations" work in light of its own actual situation and daily production safety work.
